Medical Evaluation
Complete the Form
Complete the respiratory medical evaluation questionnaire. Once cleared you will receive an email with next steps.

Training
Complete the Training
In-person respirator training must be completed through Organizational Development (aka OrgDev).

Schedule
Complete the Fit Test
Use the online self-scheduling tool linked below to schedule your in-person fit test.

Please allow 15 minutes at the start of each shift for our technician to run quality checks. You must have a signed medical evaluation prior to your appointment.
Please be sure to be CLEAN-SHAVEN when appearing for your fit test. Facial hair that does not fit under the respirator is a violation of federal law and will result in a turnaway. We will ask you to return when you are able to be fit-tested.
PLEASE DO NOT MAKE AN APPOINTMENT UNTIL YOU HAVE BEEN CLEARED TO FIT TEST.
