Medical Evaluation

Complete the Form

Complete the respiratory medical evaluation questionnaire. You will receive an email with next steps, once your questionnaire has been reviewed.

Training

Complete the Training

Go to Western State Hospital’s Online Learning Center and look for Training: DSHS BHA WSH N-95 Respirator Training OLT. *You must use a WSH computer/login to access the training.

Schedule/Drop-In

Complete the Fit Test

Please do not make an appointment until you have been medically cleared to fit test. We have locations in Gage and Bldg 8.

Fit Testing Calendar

We accept drop-ins on Tuesdays & Thursdays if you have received your medical clearance notification via the email you provided on your medical evaluation form.

No appointment is needed for Birth Month Testing!
