Thank you for your commitment to your safety. To become compliant there are 3 easy steps: Required Medical Evaluation Questionnaire (RMEQ), Training, and the In-Person Fit Test.

Training
Complete the Training
Go to Western State Hospital’s Online Learning Center and look for Training: DSHS BHA WSH N-95 Respirator Training OLT. *You must use a WSH computer/login to access the training.

Medical Evaluation
Complete the Form
Complete the respiratory medical evaluation questionnaire. You will receive an email with next steps, once your questionnaire has been reviewed.
Before starting your medical evaluation, please complete your N-95 Respirator training and have the completion date ready to enter into the form.
ATTENTION!!
You need to finish your N-95 Respirator training before you do your medical evaluation. You will need to enter the date you finished the training on your medical form. If you don't have this date, you can't complete your form. You can find your training date in the Online Learning Center.

In Person Fit Testing
Complete the Fit Test
Use the online self-scheduling tool linked below to make your appointment or select the link to view the drop-in calendar.

Please do not make an appointment until you have been medically cleared to fit test. We have locations in the Gage Clinic Area and Civil Bldg 8.
Fit Testing Calendar
WSH Hours
You are welcome to drop in if you have received your medical clearance notification via the email you provided on your medical evaluation form.
Please do not disturb the clinic workers. They are not affiliated with our program and do not have information regarding our hours or fit testing.
PLEASE DO NOT MAKE AN APPOINTMENT OR DROP IN UNTIL YOU HAVE BEEN CLEARED TO FIT TEST.
